Being faced with discharge can sometimes be a daunting prospect for some patients and families especially when a care package needs to be set up before hand. If a family is new to using a care service its sometimes hard to know what to do first. We are always happy to give families help and advice on what to do in regards to pre and post discharge.
We would be more than happy liaising with your GP, district nurses, social worker, discharge co-ordinator, pharmacist, mac nurse etc to get in place a rock solid care package that allows your life to be as fulfilling as possible.
For patients being discharged from hospital, our assessment is done in two parts:
A home visit will be arranged with yourself or your family and one of our team will undertake an assessment at your home to ensure all equipment needed for care to commence is in situ and also to make sure that your home is safe for both carer and client to function in (a now) work environment.
A hospital visit will be carried out. This will allow us to speak with your ward nurses and doctors, your family and of course yourself. An up to date medication and TTA (medication your take home) list will be documented. Any OT (Occupational therapy) and physiotherapy reports will also be documented along with all other information needed to put together an in-depth care plan that works for you, your family and care workers alike.
